Easy Booking Process - two options for booking
         Option 1

• Book a service through our website, including type of service/add on, date/time, contact information.

• Our nurse will contact you via call/text to verify appointment.

• Upon arrival, our nurse will review information and create a customer profile for you with name, email, phone number, and payment.

• Sign consent.

• Payment is processed.

• Simply call or text 850-287-5275 or Email us at acquavitahealth@outlook.com

• Please provide us with: – Name – Phone # – Address – Desired Date/Time – Desired service. We will work on accommodations and answer any questions about our services.

• Once our nurse is at your location they will create a brief profile  using the information you provided.
